Lipotropic injections vs semaglutide shots

Lipo shots mix nutrients that clear fat from the liver in animal studies. Semaglutide is a GLP-1 drug with large weight loss trials. Here is the honest comparison, and when B12 is the right call.

The verdict: For weight loss, semaglutide has the trial evidence: 14.9% average loss over 68 weeks for the brand drug in STEP 1. No lipotropic shot is FDA approved for weight loss, and a B12 shot is the right call for a diagnosed deficiency. Compounded semaglutide is not FDA approved.

Lipotropic injections vs semaglutide verdict

Lipotropic injections are sold for weight loss, yet none is FDA approved for it, and no randomized trial of them for weight loss appears in PubMed. Semaglutide is the opposite case. In STEP 1, adults without diabetes on weekly brand semaglutide 2.4 mg lost an average of 14.9% of their body weight over 68 weeks, against 2.4% on placebo (NEJM, 2021).

On lipotropic injections vs semaglutide, the drug with the evidence wins. Harbor's compounded semaglutide is not FDA approved, and STEP 1 tested brand semaglutide, not a compounded version. The guide to how semaglutide works for weight loss explains the mechanism.

One exception matters. If a blood test shows low vitamin B12, a B12 shot is the right call, and semaglutide does not treat it.

Lipo shots vs semaglutide at a glance

Lipo shots and semaglutide shots are both injections, and the likeness ends there. The table compares them on the points that decide a weight loss choice.

From the Wegovy and cyanocobalamin prescribing information, STEP 1, NIH, FDA's compounding lists and Harbor's semaglutide page, checked November 3, 2026. Trial results are for brand semaglutide. Compounded semaglutide is not FDA approved, and your clinician sets the dose.

Lipotropic injections vs semaglutide shots side by side

Lipo injections and semaglutide shots part ways on contents, testing and approval.

What is in MIC & lipo B12 injections

MIC injections take their name from three ingredients: methionine (an amino acid), inositol and choline. A review of lipotropes, substances that hasten the removal of fat from the liver, names choline, betaine, myo-inositol, methionine and carnitine as the main ones, with the effect shown in animals (Crit Rev Food Sci Nutr, 2013).

Lipo B12 injections and MIC B12 injections add vitamin B12 to the blend. No lipotropic blend is an FDA-approved product, so no label fixes its recipe or strength, and each seller decides what goes in.

A Wegovy shot holds one active drug, semaglutide, a GLP-1 receptor agonist. Compounded semaglutide is a pharmacy-made product, distinct from Wegovy and Ozempic, and is not FDA approved. The comparison of compounded semaglutide vs Ozempic sets out the differences.

How each one works

Semaglutide is a GLP-1 analogue with 94% sequence homology to the human hormone. The Wegovy label calls GLP-1 a physiological regulator of appetite and caloric intake, and semaglutide acts on its receptors in brain areas that govern appetite (Wegovy prescribing information).

Lipotropes work on a different problem: fat building up in the liver. Choline deficiency can cause liver damage and fatty liver disease, but NIH notes that frank choline deficiency in healthy men and nonpregnant women is very rare (NIH ODS). Correcting a shortage is not the same as adding more. For one serious form of fatty liver disease, noncirrhotic MASH with moderate to advanced fibrosis, brand Wegovy injection has an accelerated FDA approval (Wegovy prescribing information).

B12 matters for a different reason. A lack of vitamin B12 can cause anemia and permanent nerve damage, according to MedlinePlus.

What the weight loss evidence shows

STEP 1 enrolled 1,961 adults with obesity, or overweight plus a weight-related condition, and no diabetes. With diet and activity counseling, 66.1% of people on semaglutide 2.4 mg lost at least 10% of their weight, against 12% on placebo (Wegovy prescribing information). The maker funded the trial, which used brand semaglutide.

For lipotropic shots, the evidence is missing rather than negative. The 2013 review found human intervention studies of lipotropes practically nonexistent, and a PubMed search on November 3, 2026, found no randomized trial of lipotropic injections for weight loss.

B12 has been studied, just not for weight. NIH says it does not provide energy, performance or endurance benefits in people who already get enough (NIH ODS). Neither the cyanocobalamin injection label nor MedlinePlus lists weight loss as a use.

Prescription & FDA status

FDA approved Wegovy, brand semaglutide, for chronic weight management in June 2021. Its label covers adults with obesity, or overweight with at least one weight-related condition. No drug NIH lists as FDA approved for chronic weight management is a lipotropic or vitamin shot (NIDDK).

Compounded drugs are not FDA approved, and FDA does not review them for safety, effectiveness or quality before they are marketed (FDA). That holds for compounded semaglutide and lipotropic blends alike. FDA declared the semaglutide injection shortage resolved in February 2025, and its grace periods for compounders ended in April and May 2025 (FDA). Choline chloride and methylcobalamin sit in Category 1 of FDA's list of bulk substances still under evaluation for compounding (FDA 503A categories).

Cyanocobalamin injection, plain B12, is an FDA-approved prescription drug for B12 deficiency caused by poor absorption, including pernicious anemia (DailyMed label). The guide to NAD shots vs B12 shots covers how B12 shots are used.

Cost & how often each is given

The Wegovy label starts semaglutide at 0.25 mg once a week and raises the dose every 4 weeks toward a usual 2.4 mg maintenance dose. For compounded semaglutide, your clinician sets the dose and schedule. Harbor's program starts at $99 a month and does not rise with your dose.

Lipotropic shots have no FDA label, so no labeled dose, strength or schedule exists. The seller sets the recipe, visits and price, so ask what a full course costs first.

For a deficiency, the MedlinePlus vitamin B12 shot facts describe daily shots for the first 6 to 7 days, then every other day, then every 3 to 4 days, then monthly, sometimes for life.

Who should choose lipo injections or semaglutide shots

For weight loss, semaglutide shots are the evidence-based choice. Harbor semaglutide shots are a weekly injection at home for adults, typically with a BMI of 27 or higher, and a board-certified physician reviews your history first. If it is not the right fit, you are not charged.

Semaglutide shots may suit you when:

  • You have obesity, or overweight with a weight-related condition such as high blood pressure
  • You want a molecule tested in placebo-controlled weight loss trials
  • You want one shot a week at home, at a monthly price that stays flat as your dose rises

A lipotropic shot has no proven use for weight loss. A plain B12 shot may be the right call when:

  • A blood test shows low B12, or your clinician suspects a deficiency
  • You have pernicious anemia, or a low B12 level linked to celiac disease, Crohn's disease or past stomach or bowel surgery, including weight loss surgery
  • You eat a vegan diet or take metformin long term, and your B12 level comes back low

Semaglutide is not prescribed to anyone with a personal or family history of medullary thyroid carcinoma or MEN 2, and it is stopped in pregnancy and at least 2 months before a planned one. The cyanocobalamin label lists sensitivity to cobalt or B12 as a reason not to use it and warns about early Leber's disease. For anything else, message your Harbor clinician.

Bottom line on lipotropic injections for weight loss

Lipotropic injections for weight loss rest on animal studies of liver fat, not human weight loss trials. Semaglutide rests on large placebo-controlled trials of the brand drug, and Harbor offers it as a weekly compounded shot, not FDA approved, after a physician review.

Test your B12 before paying for any vitamin shot. If it is low, B12 is the treatment. If it is normal and weight is the goal, a GLP-1 has the evidence.

Frequently asked questions

People asking do lipotropic injections work usually mean for weight loss, and no randomized trial has shown that they do. Lipotropes such as choline clear fat from the liver in animal studies, but frank choline deficiency is very rare in healthy adults.

Not when your B12 is normal. NIH says B12 does not provide energy or endurance benefits in people who get enough, and neither the cyanocobalamin injection label nor MedlinePlus lists weight loss as a use. B12 injections for weight loss differ from B12 injections for a diagnosed deficiency, which can be needed for life. Vitamin B12 deficiency affects between 3% and 43% of older adults, so a blood test comes first.

Ask the clinician who prescribes semaglutide. STEP 1 paired semaglutide with diet and activity counseling, not with lipotropic injections, so nothing shows an added lipo shot changes the result. Tell your clinician about every injection, vitamin and supplement you take, and never stop B12 prescribed for a deficiency.
